What I check: eligibility category (c)(3)(B); the filing window (no earlier than 90 days before and no later than 60 days after your program end date, and within 30 days of your DSO's OPT recommendation in SEVIS); the I-20 OPT recommendation page and dates; name, A-number, I-94 and SEVIS ID consistency; prior EADs and employment authorization history; photo, signature and fee; and the supporting document list.
What you receive: an annotated copy of your I-765, a correction checklist, and a short written note on anything in your history that needs explaining.
Not included: filing on your behalf, representation before USCIS, STEM OPT (c)(3)(C) or pre-completion (c)(3)(A) applications, and the USCIS filing fee. This is a limited-scope document review, not ongoing representation, and no approval outcome is guaranteed.
